Social Worker Job Opportunity

Leading established non-profit medical research and healthcare organization seeks full-time Social Worker for kidney/pancreas pre-transplant program patients and families. Successful candidate will provide psychosocial evaluation, counseling and resource referral; work in collaboration with multi-disciplinary treatment team; participate in patient/family and staff orientation, education and in-service activities to promote understanding of the psychosocial impact of ESRD; document services in keeping with facility policies to meet Medicare and Quality Assurance REQUIREMENTS MSW, LCSW Required along with PC skills and 3-5 years experience in renal medicine dialysis.
